Mobile Integrated Health for COVID-19 and Stroke
The Department of Health Services (DHS), Division of Public Health (DPH) is issuing this Grant Funding Opportunity (GFO) to implement a Mobile Integrated Health for COVID-19 and Stroke program through funding by the U.S. Centers for Disease Control and Prevention (CDC) through the National Initiative to Address COVID-19 Health Disparities Among Populations at High-Risk and Underserved, Including Racial and Ethnic Minority Populations and Rural Communities.

In this project, a partnership exists between an Emergency Medical Service (EMS) provider and a local hospital. Upon discharge from the hospital, stroke patients are referred by the hospital to their partner EMS provider to perform an at-home visit within 30 days post-discharge. At this visit, patient assessments including home safety check, medication checks and primary care information will occur, along with assessment of risk for COVID-19 infection, including vaccination status and messaging reflecting current COVID-19 risk status.

The ultimate goal and purpose of this project at the DHS level is to (1) directly reach 1,000 individuals, post-stroke discharge in an effort to reduce additional strokes and COVID-19 infections, with a specific focus on those populations with higher rates of both and (2) develop methods by which this mobile integrated heath protocol can work in a variety of settings in Wisconsin and beyond.

At the local level, the goal and purpose is to facilitate relationships between EMS and hospitals that improve data collection for the purposes of improving care. A long-term vision for this project is that these relationships, and the continuity-of-care based program, will continue after the conclusion of this project period.
