Advancing Equity in Maternal and Child Health: Open-Ended Project Proposal Funding Application Survey
Anticipated Award Amount: Up to $500,000 per organization, total over eighteen months.

The Wisconsin Maternal and Child Program and the Maternal and Infant Mortality Prevention Program is making up to $6 million available over eighteen months to fund innovative projects and programs designed to disrupt inequities in health outcomes for infants, children, birthing people, and people of reproductive age. Interested applicants are asked to describe a community need or gap in services and describe the proposed project or program designed to address the need or gap. The priority areas for this investment include:

•Social Determinants of Health (such as food security, affordable housing, employment, social connections, etc.)
•Mental Well-Being
•Building Responsive and Equitable Healthcare Systems (for example, training providers or health systems on the role of doulas and community health workers, training providers or health systems)
•Perinatal Workforce Development (for example, training or reimbursement doula services or community health workers)
•Connecting Families to Information and Resources (targeted community outreach or education campaigns, related to the above priority topics)
•Wraparound support for perinatal populations in precarious situations (homelessness, incarceration, etc.)

At least five awards will be made to small community-rooted organizations. Grant funding is expressly dependent upon the availability of funds to the Wisconsin Department of Health Services (DHS) from the Health Resources and Services Administration, Title V Maternal and Child Health Grant, and the American Rescue Plan Act State and Local Fiscal Recovery Funds. Applicants will be awarded based on available funding, capacity, and ability to meet application criteria. The project period begins January 1, 2023 and ends June 30, 2024. Each contract will have a duration of one calendar year with the possibility of annual renewal for the duration of the project period, contingent on funding availability and performance. Should additional funding become available, award amounts may be altered.
Expectations for grantees:

•Selected applicants must be prepared to develop an approved budget, budget narrative, and a comprehensive work plan outlining specific project deliverables in collaboration with the Title V Program. These documents must be completed before the contract is put in place and funds can be received.

All recipients will be assigned a DHS staff contract monitor. Contract monitors will meet with all recipients on a quarterly basis (at a minimum) to discuss progress on project deliverables, how the money is being expended, and what the project outcomes are.

Award recipients will also receive technical assistance over the grant period to support the sustainability of their project or program, including training and support in program evaluation, grant writing, fiscal management, and other topics identified by grantees.

Title V Background

The Wisconsin DHS Title V Program provides funding and support to local and tribal health agencies and other organizations to support the well-being of mothers, children, and families. Funded projects will help advance the Title V Program’s priorities. The current priorities of Wisconsin’s Title V Program are:
·Advance equity and racial justice.
·Assure access to quality health services.
·Cultivate supportive social connections and community environments.
·Enhance identification, access, and support for individuals with special health care needs and their families.
·Improve perinatal outcomes.
·Foster positive mental health and associated factors.
·Promote optimal nutrition and physical activity.
