Peer Support Matching for Families of Children and Youth with Special Health Care Needs (CYSHCN) Funding Application Survey
Anticipated Annual Award Amount: Up to $150,000 per year, up to 3 years.

Funded agencies will recruit, train, and match experience caregivers to provider peer-to-peer support to other parents or caregivers.

Grant funding is expressly dependent upon the availability of funds to the Wisconsin Department of Health Services (DHS) from the Health Resources and Services Administration, Title V Maternal and Child Health Grant. Applicants will be awarded based on available funding, capacity, and ability to meet application criteria. This project begins January 1, 2023 and ends December 31, 2025. Each contract will have a duration of one calendar year with the possibility of annual renewal for the duration of the project period, contingent on funding availability and performance. Should additional funding become available, award amounts may be altered.

Minimum Expectations
Organizations receiving this award will be expected, at a minimum, to perform the following activities:
· Conduct targeted outreach to recruit parents, guardians, or other caregivers with lived experience who will offer emotional support and information to other caregivers, matched based on shared needs or experiences
· Provide evidence-based or evidence-informed training to recruited parents, guardians, or other caregivers to prepare them to provide appropriate support to their matched peers, including training on trauma-informed care
· Provide support to recruited peer mentors, aligned with the guidance and endorsed practices of Parent to Parent USA
· Maintain an organized system or database of peer mentors for matching trained mentors with other parents, guardians, or other caregivers seeking support, to ensure appropriate matching based on needs preferences, considering such factors as culture, race, language, socioeconomic status and program eligibility, disability and other child/family factors
· Promote the matching program and provide targeted outreach to parents, guardians, and other caregivers who could benefit from peer support navigating diagnoses and systems of care
· Prioritize equity and actively incorporate community voice into program planning and services

Title V Background
The Wisconsin DHS Title V Program provides funding and support to local and tribal health agencies and other organizations to support the well-being of mothers, children, and families. The current priorities of Wisconsin’s Title V Program are:
· Advance equity and racial justice.
· Assure access to quality health services.
· Cultivate supportive social connections and community environments.
· Enhance identification, access, and support for individuals with special health care needs and their families.
· Improve perinatal outcomes.
· Foster positive mental health and associated factors.
· Promote optimal nutrition and physical activity.

Funded projects will help advance the Title V Program’s priorities, and selected applicants will work with the Title V Program to develop an approved budget, budget narrative, and a comprehensive work plan that outlines specific project deliverables.
