This posting is extending the prior submission deadline of 12/9/22, extending it until 01/27/2023 with an updated attachment DMS Member Feedback RFA 12-27-22. Please submit applications and any questions to the following email address: DHSDMSMemberFeedbackAllianceRFA@dhs.wisconsin.gov The Member Feedback Alliance (MFA), part of the Division of Medicaid Services (DMS) of the Department of Health Services at the State of Wisconsin, is seeking applications from Community Based Organizations (CBOs), healthcare agencies and community groups interested in engaging with patients, clients, or individuals they serve to obtain feedback about their services and programs. The data gathered would ultimate be used to inform decision-making for program/services improvements for the grantee agency as well as for DMS.
