Wisconsin Department of Health Services Chronic Disease Prevention Program is seeking to partner with an organization which holds an umbrella license to all Self-Management Resource Center (SMRC) programs, including the Diabetes Self-Management Program (DSMP) known as Healthy Living with Diabetes or Vivir Saludable con Diabetes, and provides training, technical assistance, and support to current or aspiring Self-Management Resource Center Diabetes Self-Management Programs.
