The Wisconsin Chronic Disease Prevention Program is seeking to partner with an organization to focus on implementing sustainable policy, systems, and environmental change efforts to reduce health disparities related to access to healthy foods in communities across Wisconsin. The work will support community organizations and agencies who serve priority populations throughout Wisconsin.
